Adult Day Respite

      • Provides respite for caregivers
      • Programming tailored for clients with Dementia or Alzheimer’s
      • Educational programs designed for varying levels of participation
      • Activities include mild exercise, arts, crafts, and socialization
      • Nutritious hot lunch provided by the PPSC Café
      • Mondays, Wednesdays and Fridays 10am-2pm @ 25 Howard St
      • Case Management
      • Assessment, screening and evaluation of individual needs
      • Information and referral to community resources and programs
      • Ongoing support and advocacy from professional and caring staff
      • Voluntary contribution 

You Are Not Alone (YANA)  

      • Daily phone call from trained PPSC volunteer 
      • Engaging topics of conversation 

Café 211 

Home Delivered Meals 

      • Hot meals up to seven days a week 
      • Nutritious meals for homebound seniors through delivery 
      • Contact Case Manager to determine if you qualify  
      • Voluntary contribution $3.50-$8.00 

Counseling  

      • Our team of therapists are able to help you through major life transitions 
      • Individual and group counseling services offered in person and via Zoom 
      • We offer support in drug, alcohol, family relationships, violence issues, anxiety, depression, grief, loss and more 
      • Sliding scale available  
      • Insurance accepted 

PEARLS 

      • Program to Encourage Active, Rewarding Lives for Seniors 
      • Evidence based depression treatment program designed specifically for older adults 
      • No diagnosis required to participate 
      • Experience short term sessions with tangible results
      • PEARLS is backed by evidence based practices
      • Flexible and adaptable to meet the needs of our community 

Fallproof 

      • Evidence based balance program 
      • $10.00 per class 
      • Must have application completed and physician’s approval

Bridge the Gap 

      • Monthly Rent and/or Utility Assistance 
      • Application required 

iRide  

      • Volunteer rides provided  
      • Application required 
      • Voluntary contribution 

Caregiver Support Program 

      • Confidential and emotional support for those taking care of loved ones 
      • Group facilitated by trained therapists  
      • Education around Dementia and other related cognitive disorders 
      • 1st, 2nd, and 3rd Tuesdays of the month

S.A.F.E Team – Crisis Intervention Program  

      • Mental health assessments and referrals 
      • Counseling request 
      • Medical assessments and first aid care 
      • Dispute mediation 
      • Transport to staffed services

Petaluma Fair Housing Counseling  

      • Helps any person in Petaluma who has been discriminated against in the rental, sale, financial or appraisal of housing 
      • Investigates complaints and coordinates referral services to assist individuals when their rights under the State and Federal Fair Housing laws have been violated

Homeless Prevention 

      • City of Petaluma rental assistance – provides payment for tenants in Petaluma to the landlord in the event of a tenant default or move-in deposits 
      • Bridge the gap – program provides Petaluma residents who are 60 and older small monthly payments to landlord to allow them to stay in their homes
