Founded in 1996, the Petaluma Health Center (PHC) is a non-profit Federally Qualified Health Center (FQHC), committed to providing high quality health care with access for all in Southern Sonoma County. PHC has built a strong reputation for its innovation and has been on the leading edge of providing comprehensive primary health care in a team based, patient centered system, emphasizing overall wellness and the value of the patient provider relationship. PHC takes a whole person approach to keeping people healthy, providing medical, dental and mental health care services. In addition to providing health services at their main clinics, the Petaluma Health Center operates an onsite clinic located at the Mary Isaak Center emergency shelter, accessible to COTS’ residents and the unsheltered community. Nurses, Nurse Practitioners, and Nurse Practitioner Residents provide services staff the Mary Isaak Center clinic. They can triage acute health issues, administer TB tests, help with health system navigation, check insurance, make appointments, send messages to providers, and more. Petaluma Health Center clinic staff are key partners in the COTS Recuperative Care program at Mary Isaak Center, which offers a safe place for people who are homeless to recuperate after they are discharged from hospital stays or emergency rooms. Recuperative Care is a short-term, residential program that provides individuals experiencing homelessness a safe, welcoming place to heal from illness, injury, or surgery. With onsite health care coordination and intensive case management, Recuperative Care focuses on health stabilization, promoting client self-sufficiency, and supporting clients’ health and housing goals. Over 85% of people move from COTS Recuperative Care into permanent housing.
